What to send with the referral

App preparation checklist. Send available information through approved clinical channels; missing items should not automatically delay review.

Pathology report(s) and, if possible, the biopsy slides/blocks for expert dermatopathology review.
Results of any TCR gene rearrangement (clonality) studies.
Available laboratory and blood flow-cytometry results, if obtained; this checklist does not order tests.
Clinical photographs documenting extent and morphology of lesions.
Body surface area / mSWAT estimate and a brief treatment history (what was tried and the response).
